EORTC Account Request form

Please, fill-in the form and submit it to send us your details needed to create an EORTC account.

This account can be used to register patients with ORTA, download full protocols in pdf, se VISTA Remote Data Capture (for a limited number of protocols) or access the www.eortc.be /members area to download the SUSARs of your studies.

The accounts are limited to people involved in EORTC trials and working in EORTC institutions

Warning

If you don't know your institution number, please contact the Data Manager of the protocol before filling your request
No follow-up will be given if we are unable to double check your details with our records (institution, protocol, ...) so give as much information as you can in the comments box.
The accounts are created about once a week. You will receive the confirmation by email
